LifeStream Services is the Aging and Disability Resource Center for East Central Indiana.
LifeStream serves individuals in Blackford, Delaware, Fayette, Franklin, Grant, Henry, Jay, Madison, Randolph, Rush, Union, and Wayne counties in Indiana.
Indiana PathWays for Aging Updates
As of July 1st, Indiana launched PathWays for Aging, a collaborative effort between the Family and Social Services Administration (FSSA) and health plan providers, to enhance long-term services and supports for individuals aged 60 and over. The PathWays for Aging program will provide beneficiaries with both a care coordinator and a service coordinator through the oversight of a contracted health plan. These health plans, or “managed care entities” are Anthem Blue Cross and Blue Shield, Humana Healthy Horizons in Indiana, and UnitedHealthcare Community Plan of Indiana.
Upon the launch of PathWays for Aging on July 1, LifeStream Services and your LifeStream care manager are no longer a part of your care team through these contracted health plans. Your care and service coordination has transitioned to the managed care entity that you selected during open enrollment or were automatically enrolled with to align with your current D-SNP plan.
