Indiana PathWays for Aging Updates

Indiana is preparing to launch PathWays for Aging, a collaborative effort between the Family and Social Services Administration (FSSA) and health plan providers, to enhance long-term services and supports for individuals aged 60 and over. The PathWays for Aging program will provide beneficiaries with both a care coordinator and a service coordinator through the oversight of a contracted health plan. These health plans, or “managed care entities” are Anthem Blue Cross and Blue Shield, Humana Healthy Horizons in Indiana, and UnitedHealthcare Community Plan of Indiana.

Upon the launch of PathWays for Aging on July 1, LifeStream Services and your LifeStream care manager will no longer be a part of your care team through these contracted health plans. Your care and service coordination will transition to the managed care entity that you selected during open enrollment or were automatically enrolled with to align with your current D-SNP plan.

For questions about your services on or after July 1, contact Indiana PathWays for Aging at 877-284-9294.
