SUMMARY
Responsible for assisting care managers and the ADRC supervisory team by providing clerical support, securing information, monitoring client satisfaction with services and client advocacy.
ESSENTIAL DUTIES AND RESPONSIBILITIES

Clerical Support
- Fax information and care plans to providers and ensure receipt of paperwork by provider
- Make necessary phone calls to follow up with providers, physician offices, and other community agencies to obtain necessary information/paperwork to carry out care plan
- Assist with maintaining client files accurately and according to agency and State standards
- Answer incoming telephone calls for offices as assigned
- Assist supervisory staff with meeting and training set up and documents
Client Services
- Contact clients by phone to schedule appointments, follow up on outstanding case management requests, and inform clients and staff of service activities on a timely basis
- Collect information on the client’s health and functional status accurately and timely by completing care plan monitors as assigned for T3, SSBG, and CHOICE clients according to program timeframes
- Assist with obtaining prescriptions, provider bids and other required documents for request for approvals (RFAs) for client home modifications and specialized medical equipment
- Follow agency and program documentation standards to document all client activity within the appropriate timeframes
- Be certified as a SHIP Counselor and provide assistance with calls and walk-ins
- Gather information from walk-ins and ensure timely referral to I&A or to an Options Counselor for assessment
Client Advocacy
- Advocate on behalf of client to ensure access to community services, entitlements and benefits
- Maintain relationships with community resources and have working knowledge of services available to clients

EDUCATION AND/OR EXPERIENCE
Associate’s degree in human services or related field; or a High school diploma/GED with one (1) or two (2) years’ experience and/or training in a related field; or an equivalent combination of education and experience.
MATHEMATICAL SKILLS
Ability to apply basic mathematics, algebra, and geometry.
REASONING ABILITY
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving concrete variables in standardized situations. Conflict resolution skills.
COMPUTER SKILLS
To do this job successfully, an individual must have working knowledge of Microsoft applications, and to be able to quickly learn software programs for various home and community based programs.
CERTIFICATES, LICENSES, REGISTRATIONS
Must possess a valid driver’s license. Must be covered, maintain coverage and provide documentation of current automobile insurance meeting LifeStream’s amount requirement. Must be able to travel throughout the LifeStream service area independently and beyond the service area on occasion. Must be able to be certified in SHIP counseling.

SUMMARY
Responsible for assessing client needs providing information and referral services through the Aging & Disability Resource Center, writing and monitoring care plans, and utilizing resources within guidelines. Work involves information and assistance, case management assistance and community relation development.
ESSENTIAL DUTIES AND RESPONSIBILITIES

Information and Assistance
- Answer I&A calls, determine needs and document call specific data in REFER software
- Appropriately refer callers to resources available, including private pay and LifeStream funded programs, within appropriate time frames
- Conduct intake and initial assessments for LifeStream funded case management programs
- Place individuals on waiting lists for LifeStream programs as needed and do monitoring of all clients on a waiting list for LifeStream services
- Follow QA procedures for I&A including documentation of follow up calls
- Be certified as SHIP counselor, CIRS-AD through AIRS, and as case manager with Division of Aging
- Assist with maintaining resources in the I&A database
- Provide adequate supply of both LifeStream program and external program brochures and information to supply to callers/walk-ins
- Maintain client confidentially and follow HIPAA guidelines
Case Management
- Conduct initial assessments to determine client needs in the client’s home
- Assess client situations to determine need and provide resources as appropriate
- Develop care plans for assigned clients including informal support
- Complete PASRR cases/Options counseling as assigned
- Develop care plans for assigned clients including informal supports
- Monitor care plans as required by funding source(s)
- Accurately record time and tasks according to agency and funding guidelines
- Display competence for all guidelines, rules, and instructions for Medicaid Waiver, CHOICE, SSBG, Title III, PASRR, ADRC and Caregiver funding
- Assist in training ADRC staff
- Prepare daily, monthly, yearly scheduling system to monitor due dates of assessments, reassessment and updates
- Assist with identifying participants in grant projects or special programs
- Assist other ADRC departments as needed
- Attend all scheduled training/meetings including State licensure/certification
Community Relations
- Develop and maintain working relationship with area organizations and service providers
- Follow agency customer service standards
- Respond to community based referrals within 24 hours
- Receive and respond to walk-ins
- Staff area health fairs as needed
- Provide information to medical offices and businesses about ADRC programs
QUALIFICATIONS

EDUCATION AND/OR EXPERIENCE
Bachelor’s degree in social work, sociology, counseling, psychology, gerontology, a valid IN RN license or a Bachelor’s degree in any field with a minimum of two (2) years full-time, direct service experience with the elderly or disabled which includes assessment, care plan development and monitoring; or a Master’s degree in a related field may substitute for the required experience. Must meet experience requirements outlined by the Division of Aging for A & D Medicaid Waiver, SSBG/Title III, CHOICE, PASRR, and TBI Waiver. Must be able to be certified in SHIP counseling and CIRS-AD. LSW preferred.
LANGUAGE SKILLS
Ability to read, analyze, and interpret documents, procedure manuals, legal documents and to understand correspondence. Ability to respond to common inquiries or complaints from clients, regulatory agencies, or members of the business community. Ability to effectively present information to top management, and public groups.
MATHEMATICAL SKILLS
Ability to calculate figures and amounts such as percentages. Ability to apply basic mathematics, algebra and geometry.

OTHER SKILLS AND ABILITIES
Knowledge of medical terminology and ability to interpret medical charting. Working knowledge of community resources. Ability to complete, organize paperwork that is thorough and accurate. Ability to work and travel independently throughout the service area. To perform this job an individual must have knowledge of Microsoft applications and InSite and REFER database.
