Current Open Positions:

Responsible for assessing client needs, writing and monitoring care plans, and utilizing resources within guidelines.  Work involves case management, resource development and management, and community relations development.

Behavioral Health

  • Address challenges faced by individuals with behavioral health related to social isolation
  • Assess social determinants of health and transition between settings
  • Assist with accessing services for individuals
  • Assist ADRC staff with individuals in challenging situations
  • Assist with training to enhance ADRC services with clients with behavioral issues
  • Build relationships with CMHCs to expand and enhance access services for older adults and people with disabilities

Case Management

  • Conduct reassessments to determine client needs
  • Develop care plans for assigned clients including informal supports
  • Monitor care plans on the schedules indicated by funding sources
  • Complete adult and child protective service referrals
  • Adjust care plans based on reassessment results, available resources, client eligibility and family support
  • Maintain client documentation and files according to program/agency standards
  • Prepare daily, monthly, yearly scheduling system to monitor dates of assessments, reassessments, and updates
  • Maintain strict confidentiality
  • Strictly follow HIPAA guidelines
  • Demonstrate knowledge of the interrelated functions of I&A services and case management
  • Implement care management services timely for all client referrals
  • Attend all scheduled training/meeting including state licensure/certification
  • Assist with identifying participants in grant projects or special programs
  • Keep up to date on new developments in the field of aging and governance
  • Assist other ADRC departments as needed

Resource Development/Management

  • Accurately record time and tasks according to agency and funding source guidelines and time frames
  • Display competence for all guidelines, rules/instructions for Medicaid Waiver, CHOICE, SSBG, Title III, Caregiver
  • Promote private pay services for all programs sponsored by LifeStream
  • Assist in training new care managers
  • Share resource information with co-workers
  • Make referrals to necessary agencies for services including durable medical equipment, mental health services and hospice

Community Relations

  • Develop and maintain working relationships with area organizations and service providers
  • Demonstrate agency’s customer service standards
  • Conduct outreach activities for service referrals
  • Participate in advocacy activities as assigned

 

EDUCATION AND/OR EXPERIENCE

Bachelor’s degree in social work, sociology, counseling, psychology, gerontology, a valid IN RN license or a Bachelor’s degree in any field with a minimum of two (2) years full-time, direct service experience with the elderly or disabled which includes assessment, care plan development and monitoring; or a Master’s degree in a related field may substitute for the required experience.  Must meet experience requirements outlined by the Division of Aging for A & D Medicaid Waiver, SSBG/Title III, CHOICE, and TBI Waiver.  LCSW is preferred.

Must meet case management certification requirements, and be approved for CHOICE, SSBG/Title III, A & D Medicaid Waiver and TBI.  Must possess a valid driver’s license.  Must be covered, maintain coverage and provide documentation of current automobile insurance meeting LifeStream’s amount requirement.

SUMMARY

The Dementia Outreach Specialist (DOS) role is responsible for implementing the State of Indiana’s Dementia Care Specialist Program (DCSP) at LifeStream Services. Towards improving Indiana’s dementia-related population health outcomes, the DCSP supports Indiana’s transformation into a dementia capable state equipped with dementia capable care and services for all Hoosiers. Careful attention shall be focused on strategies to decrease health disparities concerning dementia in Indiana. The DCSP will improve quality of life, care, and services for persons living with dementia and their caregivers through the following essential duties and responsibilities.

ESSENTIAL DUTIES AND RESPONSIBLITIES

Training and Development

  • Serve as a subject matter expert in dementia and the dementia service network.
  • Obtain certification in an evidence-based dementia certification curriculum as approved by the DCSP.
  • Conduct an internal agency dementia staff education assessment.
  • Provide ongoing support, education, and training on dementia consistent with the DCSP guidelines to internal agency staff based on the assessment’s outcomes, and collect all pre- and post-tests and evaluation forms.
  • Maintain current knowledge of dementia, research findings, new evidence-based interventions, and other innovations for people living with dementia and their caregivers and the dementia service system at large.
  • Complete time reporting, collect and report program data, and contribute to DCSP evaluation.

Community Education

  • Conduct a community assessment resulting in the documentation of dementia services, resources, and gaps in the agency’s planning and service area.
  • Provide culturally competent public outreach and awareness about the agency and available dementia services.
  • Serve as the agency representative at community activities, health fairs, public meetings and other similar functions. Be able to speak/present comfortably at these events.

Partnerships and Engagement

  • Develop relationships to establish clinical coordination, care coordination, and community supports referral protocols among health care providers, including hospitals and physician groups, emergency and first responders, dementia diagnostic clinics, service providers, dementia organizations, and other community stakeholders.
  • Provide support, education, and training on dementia to the general public and staff of other community-based organizations and health care providers consistent with DCSP guidelines, and collect pre- and post-tests and evaluation forms.

EDUCATION AND/OR EXPERIENCE

High School Diploma/GED required. Associates degree or two years of dementia-related experience is preferred. Experience working with older adults in healthcare or community settings and/or long-term services and supports system is preferred. Experience in public health and/or health education is preferred.

LANGUAGE SKILLS

Ability to read and interpret documents, procedures manuals, and to understand correspondence.  Ability to write documents, procedures, correspondence, memos, and presentations.  Ability to convey group presentations, and counsel one-on-one. 

MATHEMATICAL SKILLS

Ability to determine budgets.  Ability to calculate figures and amounts such as percentages.  Ability to apply basic mathematics, algebra, and geometry.

REASONING ABILITY

Ability to apply commonsense understanding to carry out instructions furnished in written, oral, or diagram form.  Ability to deal with problems involving several concrete variables in standardized situations.

COMPUTER SKILLS

To perform this job successfully, an individual should have knowledge of Microsoft applications, and be able to quickly learn other software programs for various home and community-based programs for efficient and accurate program data reporting.
