LifeStream Services currently works with over 60 non-waiver providers who assist with services such as homemaking, home health aide, nursing and supplies.
Below are the application materials to apply to be one of LifeStream’s non-waiver (Choice, SSBG, Title III) providers.

Completed documents can be emailed to tsuttle@lifestreaminc.org or mailed to LifeStream at 1701 S Pilgrim Blvd. Yorktown, IN 47396.
We will review the applications to get them approved and updated in our system as soon as possible. Potential providers may email tsuttle@lifestreaminc.org for contract information for FY2027.
