LifeStream provides a variety of nutrition programs including senior community cafés, home delivered meals, and Senior Farmers Market Nutrition Vouchers.

Senior Cafés

LifeStream has 21 senior community cafés throughout 12 counties in Indiana. Lunch is provided on a donation basis (suggested donation of $2.50) to those who are 60 years of age and over and their spouse. Meals are provided to those below the age of 60 for a small fee of $6.50. Reservations to eat lunch at a café must be made at least one business day in advance by calling the site you plan to attend or call 800-589-1121.

The café sites provide more than just a meal; it is an opportunity to be active and engaged in the community.

WHAT’S FOR LUNCH?

Here’s what we’re serving at congregate sites this month. If you’re on the home delivered program these will be delivered a week later. Menus may be subject to change. Please call 800-589-1121 if you have any questions.

PLAN YOUR VISIT

Are you ready to enjoy lunch at one of our senior cafés? If you are attending one of our cafés for the first time, please fill out the form below or call 800-589-1121. Your reservation must be made at least one business day in advance. Please note that completing the intake form does not reserve your meal order. You will need to call LifeStream’s office to reserve your meal.

Home Delivered Meals

LifeStream can deliver meals right to your door if you live in our 12 county service area. Home-bound individuals age 60 years and older might qualify to receive a meal at home on a donation basis.

Meals are available for anyone for $6.50.

Nutrition RFP

LifeStream Services is currently seeking proposals in response to the 2024-2028 RFP for congregate and home-delivered nutrition services. Bids are due on or before 5:00pm Eastern Standard Time on November 22, 2023. Nutrition RFP Questions and Responses:

Are meal sites operated by life stream managed employees or volunteers?

  • The meal sites are operated by LifeStream. The caterer is responsible for hiring, supervising and managing the paid staff where volunteers are not available with LifeStream support.

Are all meal sites able to do onsite retherm and receive weekly food deliveries of the meals and store them to prepare the day of service?

  • All meal sites with the appropriate equipment and security would be available for weekly rethern service

In regards to the bid bond, are you requiring 5% of the total amount of 4 years worth of meals served and then a performance bond of 1/12 of the total amount of meals served over a 4 year period?

  • Yes, that is correct for a 4 year period.

Are we allowed to bid only on the Home Delivered Meals portion and not the congregate?

  • The contract is for congregate and home delivered service.

Will the reimbursement rate be based off the Medicaid Waiver rate, or are we required to submit a proposed reimbursement rate?  If so, is there a requirement for a contribution amount on the bid?

  • The contract is not for Medicaid Waiver meals so you are requited to submit a proposed reimbursement rate. There is not a requirement for the contribution amount.

Are we required to send 7 copies of all supporting documents (financial statements, menus, etc.) with each of the 7 proposal copies?

  • Yes 7 copies including supporting documents.

Per page 56 (Attachment J), we want to confirm what we received is the “application/proposal packet” and there isn’t further action needed to obtain the documents required to submit the RFP.  Also, there is a due date of November 13, 2023, we want to confirm the due date is November 22, 2023.

  • Yes, you have received the application/proposal packet. Confirming the due date is November 22, 2023.

Senior Safety Net

LifeStream Services partners with Second Harvest Food Bank to offer monthly food distributions for those who are 60 and over. Proof of residence and ID are not required. Senior Safety Net is currently offered at the following locations:

Fourth Monday of the month at Visiting Angels located at 110 E. Hartman Rd. Anderson from 3pm to 4pm.

Fourth Tuesday of the month at Glad Tidings Church located at 1712 N. Walnut St., Hartford City, from 2pm-3:30pm.

Third Thursday of the month at Daleville Town Hall located at 8019 S. Walnut St. Daleville from 12:30 to 1:30pm.

Fourth Thursday of the month at Muncie-Delaware County Senior Center located at 2517 W. 8th St. Muncie from 3pm to 4pm.

First Wednesday of the month at the Marion-Grant County Senior Center located at 503 S. Gallatin St. Marion, IN 46953 from 1:00pm to 2:00pm

Third Thursday of the month at Salvation Army of Henry County located at 2500 Washington St. New Castle from 1pm to 2pm.

First Tuesday of the month at Asbury United Methodist Church located at 204 E. Arch St. Portland from 11:30am to 12:30pm.

Third Tuesday of the month at Parker City United Methodist Church located at 305 S. Main St. Parker City, IN 47368 from 3pm to 4pm.

Farmers Market Vouchers

Available June-September.

The Senior Farmers’ Market Nutrition Program (SFMNP) provides fresh, unprepared, locally grown fruits, vegetables, and edible herbs to income-eligible seniors who are over the age of 60.

Those who are interested in applying for vouchers or learning more about this program should contact Dana Pierce, Nutrition Administrator, at (765) 759-1121 ext. 231  or Micole Leverette, Community Services Administrator, at 765-620-9907. If your call is not answered, please leave a message and your call will be returned in the order in which it was received. If you are approved, the vouchers will be mailed directly to your residence.

Those that are eligible for this program will receive SFMNP electronically to buy fresh, locally grown produce from authorized growers at authorized locations. Funds will be accessible on MyMarketLink app. The app will provide a map of participating farmers markets. At this time we don’t have information on participating markets or vendors in our area.

Participants will only have 30 days from issuance to spend the $50 benefit on produce. No exceptions for any reason.
