Researchers at the IU Indianapolis Richard M. Fairbanks School of Public Health are conducting a research study about social needs among older adults.  A better understanding of the occurrence of social needs could support more effective services.

You are eligible for this study if you are 66 years of age or older, are an Indiana resident, are a current IU Health or Eskenazi Health patient, and have a traditional Medicare or Medicare Advantage plan. Participants are being asked to complete an online survey regarding social needs.

If you agree to participate in the study and complete the online survey in its entirety, you will receive a $20.00 gift card from IU Indianapolis Richard M. Fairbanks School of Public Health as a thank you for your time and effort.  There is no penalty if you do not choose to participate.

This study has been approved by the Institutional Review Board at Indiana University and is funded by the National Institutes of Health’s National Institute of Aging. If you would like to participate, please use the link below to complete the online consent form and survey.

Please ensure you:

  • Are ≥66 years old
  • Are Medicare beneficiaries
  • Have received care from IU Health or Eskenazi in the last 6 months
  • Are able to respond to a survey in English or Spanish (with support from a legally authorized representative if needed)
