Jenny Hamilton, MBA

President/CEO

Jenny Hamilton has over 24 years of experience working in long term services and supports for seniors and individuals with disabilities.  She also led the East Central Indiana Regional Collaborative to work on quality assurance in skilled nursing facilities. Jenny serves on the Board of Directors for the Indiana Association of Area Agencies on Aging and is board president of enlivIN, formerly the Indiana Aging Alliance. Jenny was honored by Anderson University with the Falls School of Business MBA Distinguished Servant Leader Award in 2012. She is a 2016 graduate of the Leadership Institute through the National Association of Area Agencies on Aging. She holds a Bachelor’s Degree in Sociology from Ball State University and a MBA from Anderson University. Jenny also received a certificate in non-profit executive leadership from Indiana University. Jenny has worked in program development and supervision for over 20 years with LifeStream and currently serves as the President/CEO for LifeStream Services.

Carissa Kirkpatrick

Vice President of Finance

Carrissa joined LifeStream Services in 2019, and brings more than 15 years of experience and diverse accounting background to organization.  She has worked in a variety of industries, including public accounting, multi-state retail and convenience store/retail fuel. 

Carrissa attended Ball State University, where she earned both a Bachelor’s of Science and Master’s of Science in Accounting.

Mandy Williams

Vice President of Programs

Mandy holds a bachelor’s degree from DePauw University, a certificate in Nonprofit Management from the University of Illinois-Chicago, and a Master’s degree in Leadership from Duquesne University. Mandy joined LifeStream Services in 2019, having most recently served as the Community Engagement Director for Joy’s House Adult Day Service in Indianapolis. She is a 2017 graduate of the LeadingAge Indiana “Leadership VIP” program, and in 2019 completed the Academy for Community Leadership through the Muncie-Delaware Chamber of Commerce and Ball State University. Mandy was a 2022 Fellow with the Indiana Rural Health Association, and received the Certificate in Fundraising Leadership (CFRL) credential from The Fund Raising School at the IU Lilly Family School of Philanthropy. 

With over 15 years of experience in education, training, marketing, and outreach, Mandy oversees the development and delivery of LifeStream’s programs, including client services, community programs, and marketing and development. She serves as the Alzheimer’s Association Greater Indiana Chapter Heartland Walk to End Alzheimer’s Chair and attended the 2023 Alzheimer’s Association Community Leaders Summit in San Diego, CA, and is a Dementia Champion with Dementia Friends Indiana.

Megan Velasquez

Director of Client Services

Megan holds a bachelor’s degree in Psychology from Ohio University and a master’s degree in Gerontology from Ball State University. Megan started with LifeStream Services in 2007. Megan is a certified resource specialist – aging and disability (CRS-AD) and a certified resource specialist- database curator (CRS-DC) through the Alliance for Information and Referral Systems. She has worked with the ADRC to become accredited through the Alliance for Information and Referral Systems (AIRS) and the National Committee for Quality Assurance (NCQA).  Megan is also a National Certified Guardian with the National Guardianship Association.  Megan oversees care management, information and assistance, options counseling, and guardianship programs.

Beth Evans

Director of Community Services

Beth has nearly 28 years of experience in the Aging Network. Her work experience includes nutrition services, volunteer management, event planning, fundraising, evidence based wellness programs, caregiver services, senior employment, and bill payer services with Area 9 In-Home and Community Services Agency and Friends Fellowship Community.

Beth holds a Certificate in Hospitality and Tourism and is certified in Evidence Based Wellness Programs and Nutrition. She is a member of Systems of Communities and Community Consortium Task Force and volunteers with Centerville Schools.

Beth oversees LifeStream’s Community Services including nutrition, wellness programs, volunteers, and event planning.

Lisa Suttle

Vice Chair

Lisa Suttle is the Regional Vice President of Clinical Services for Delaware and Madison Counties at Meridian Health Services. Meridian Health Services is a regional private, not-for-profit health care system specializing in “whole person” health, integrating physical, mental and social well-being. Prior to her employment with Meridian Health Services, Ms. Suttle worked at Universal Health Services as a Chief Nursing Officer, Reid Health as Director of Psychiatric Services/Strategic Initiatives, and Richmond State Hospital as Service Line Director. She has been in the health care industry since 1986.

Ms. Suttle received her Registered Nurse license from Indiana University East and Master of Science degree from Indiana Wesleyan University. She is a board certified Psychiatric-Mental Health Nurse through the American Nurses Credentialing Center (ANCC).

Andy Campbell

Secretary/Treasurer

Andy Campbell is a senior consultant at Finvi, LLC, a premier provider of enterprise technologies that streamline and accelerate revenue recovery for clients across healthcare, government, accounts receivable management, and financial institutions. Andy received his B.S. in Management and his Master of Business Administration degrees from Ball State University.

Andy has previously volunteered with Riverside United Methodist Church, Delta Little League, The Compass United Methodist Church, and the Delta Track Boosters Club.

Andy and his wife, Angie, attend Union Chapel and have lived in Muncie, Indiana, for twenty-five years.

Ami M. Huffman

Member

Ami Huffman is director of community development for the Jay County. She is involved in several community organizations include Jay County Community Center board of directors, Jay County Chamber of Commerce board of directors, Jay County 20/20 Vision Implementation Committee member and Portland Rotary Club, where she is vice president. She is also a member of the Indiana OCRA Certified Grant Administration. In addition, she is on the Jay Christian Academy Parent Advisory board. Her other community involvements have included Blackford County YMCA, LifeStream Advisory Committee, Tri Kappa (Beta Chapter), Jay County Fair and West Jay Community Center soccer team coach.

John W. Longnaker III

Member

John Longnaker is an attorney in Anderson, IN, where he practices in the areas of estate planning and administration, guardianships, elder law, special needs, veteran’s benefits, Medicaid planning, long term care planning, adoptions, real estate and business. He has spoken on estate planning, probate administration and elder law issues for attorney, professional and community groups. He received his bachelor’s degree from Wabash College and his J.D. degree from Indiana University School of Law. He is a Certified Elder Law Attorney (CELA) by the National Elder Law Foundation and is an accredited attorney by the Veteran’s Administration. He is also a member of the Indiana State Bar, Elder Law & Probate Trust and Real Property sections, the National Academy of Elder Law Attorneys (NAELA), and the Indiana Chapter of NAELA (Board of Directors). His community involvement includes the Anderson Rotary Club and Madison County Cancer Society. He is also a member of the Board of Elders, Bethany Christian Church.

Dr. Pierre Atchade

Member

Dr. Atchade is an Adjunct Professor at Indiana Wesleyan University. He is a member of the Garden at Gethsemane United Methodist Church, as well as Benin Diaspora USA (nonprofit organization), and a member of the World Financial Group. He has helped plan and coordinate International Friends of Garden at Gethsemane and Unity in Diversity Group – a Muncie city-wide festival. He has served as a Chairman on Allodo Management Council, a Chicago based nonprofit organization and was an honorary President of Beninese Diaspora of the USA.

Rev. Dr. Anita Jack

Member

Rev. Dr. Anita Jack served on the Board of Elders at East Side Church of God for several years, and accepted a position on Pastoral Staff in Congregational Care Church Life at East Side Church of God Anderson in 2009. Rev. Dr. Anita has been attending East Side with her husband Alonza since 2000. She was ordained as a Minister State of Indiana through Indiana Ministries of the Church of God in 2008.

Rev. Dr. Anita is a graduate of Indiana Christian University with Degree in Biblical Studies Associated of Arts Degree of Christian Ministry, Bachelors of Arts Degree of Christian Ministry, Master of Arts Degree of Christian Ministry, and Masters of Arts of Divinity,

Her other accomplishments include: She is a graduate of Harvest Bible College Doctorate of Ministry, Graduate of Leadership Academy of Madison County, Certified Stephen Ministry, Graduate of Ministers of Care Program of Saint Health System Center of Spiritual Care and, Institute for Servant Leadership Doctrinal Track Midland, Michigan, Member of the Anderson Christian Clergy, Church Women United Anderson Area 8, Member of the Christian Women Connection, Member Anderson Black Chamber of Commerce, Saint Vincent Regional Hospital Victoria Guild Board Anderson, Indiana, Founder of Grace and Truth Women’s Ministry, SHAPE Sustain Health and Purpose Excellent of Pastors. Retired for General Guide Lamp in 2003 Quality Inspection.

David Karnes

Chairperson

David Karnes was born in Norfolk, Virginia and he graduated from Tabernacle Baptist School, Virginia Beach, Virginia. Mr. Karnes received a Bachelor of Arts degree in Pre-law from Bob Jones University in 1996. He received his juris doctorate from the Indiana University School of Law in Indianapolis, cum laude, in 2001. David is a partner in the law firm of Dennis, Wenger & Abrell, P.C. in Muncie, Indiana. He is admitted to practice in the state of Indiana, as well as the U.S. District Court for the Southern District of Indiana, the U.S. District Court for the Northern District of Indiana. Professional associations include the Muncie Bar Association (Board of Directors 2008 – 2014, President 2013-14); Indiana State Bar Association (Probate, Trust & Real Property, Business Law, and Real Estate Sections), American Bar Association (2001-2016), and the East Central Indiana Estate Planning Council. David was named the Muncie Area’s Finest Business Attorney by the Star Press in 2008.

David has been highly involved in the Muncie community since moving here in 1996. He is a member of Grace Baptist Church, Muncie, Indiana, and is involved in both children and adult ministries, has served as a deacon for many years and has held the office of Deacon Chairman for several years. David completed the Academy for Community Leadership (Graduate 2004), has been a member of the Business Network International-Partners for Profit (BNI) chapter in Muncie since 2003, and has served on numerous non-profit boards. United Way of Delaware County, Inc. (Board of Directors 2010 – 2015); Hillcroft Services, Inc. (Board of Directors 2007 – 2014; Chairman 2010-11); Isanogel Center, (Board of Directors 2003 – 2006)

Adrian Moulton

Member

Adrian Moulton is the Superintendent of Monroe Central School Corporation. Mr. Moulton received a Bachelor of Science degree in Social Studies Secondary Education from Taylor University in 1995. He received his Masters in Educational Administration in 2002 from Ball State University and a Specialist in Educational Administration and Supervision Degree in 2017 from Ball State University.

Adrian is a member of the Parker City United Methodist Church and serves as the Chairperson of the Church Council. He is a board member on the Randolph County Area Chamber of Commerce and The Randolph County Area Planning Commision. Adrian is a member of the Indiana Association of Public School Superintendents and the American Association of School Administrators.

Adrian and his wife Lesley are lifelong residents of Farmland, Indiana.

Max Richardson

Member

Max Richardson is the Administrator at the Waters of Hartford City Skilled Nursing & Rehabilitation Center. The Waters of Hartford City is a National Quality Award Winning Five Star nursing home rated by Medicare. Prior to his employment with The Waters, Max worked with several Miller’s Merry Manor facilities in various roles while obtaining his Health Facility Administrator License. Currently, he is serving as the Vice President of the Hartford City Chamber of Commerce, Board Member of the Blackford County Economic Development Corporation, Hartford City Rotary Member, and serves as an Advisory Board Member for various other organizations.

Max is a graduate of Indiana University Indianapolis (IUI) with a Degree in Health Services Management. He receives a Masters of Healthcare Administration in 2024 from IUI.

Stacy Hammons

Member

Stacy Hammons is the Executive Director for Diversity, Equity, and Inclusion at Community Health Network. In that capacity, she serves as the ADA Coordinator for the Network. She also oversees Community’s supplier diversity program.

Prior to coming to Community Health Network, she worked for over 30 years in higher education, most recently as Provost and Professor of Sociology at Indiana Wesleyan University.
