INDIANA AGED & DISABLED WAIVER An amendment to the Aged & Dissabled home and community based waiver has been approved with an effective date of March 01. 2012. Please be assured the changes in this admendment do not require a change to any individuals's current plan of care.

The Senior Study Project conducted by LifeStream Services, Fisher Institute for Wellness and Gerontology and the United Way of Delaware County focuses on needs in the areas of transportation, housing and healthcare/wellness in Delaware County, IN. The study involved more than 1,000 individuals, businesses, focus groups and public sessions. The WHITE PAPER on the study is now available for review, discussion and distribution.
