Care Management Positions Open at LifeStream Services

LifeStream Services is now hiring two In-Home Care Managers and one Care Management Assistant. Below are brief job descriptions for each of these positions, as well as where each position will be located.

Care Management Assistant: This position is responsible for assisting care managers and the ADRC supervisory team by providing clerical support, securing information, monitoring client satisfaction with services and client advocacy.  Work involves clerical support, client services, and client advocacy. In-Home Care Manager: This position is responsible for assessing client needs, writing and monitoring care plans, and utilizing resources within guidelines.  Work involves case management, resource development and management, and community relations development.
